Job Overview
We are seeking a dynamic and compassionate Care Coordinator to join our dedicated care team. In this vital role, you will oversee the coordination of care services, ensuring that clients receive personalized, high-quality support tailored to their individual needs. Your will drive the development and implementation of comprehensive care plans, fostering a safe, respectful, and nurturing environment for all clients. Responsibilities
- Develop, review (quarterly), and update personalized care plans & assessments for clients, ensuring they reflect current needs and preferences
- Coordinate and communicate with healthcare professionals, social workers, families, and care staff to deliver seamless and effective care services in the form of phone calls, emails and more
- Actively applying and responding to care plan referrals when required
- Supervise care staff to ensure adherence to care protocols, safety standards, and regulatory requirements
- Manage daily rotas for care delivery, including medication administration, activities, and health monitoring
- Maintain accurate documentation of resident progress, incidents, and care activities using IT systems
- Managing, collecting, filing of E-Signatures monthly
- Facilitate staff supervision times and spot-checks, ensuring care quality is maintained
Requirements
- Understanding of care planning processes and senior care best practices
- Excellent communication skills to liaise effectively with residents, families, healthcare professionals, and team members
- Competency in using IT systems for documentation and reporting purposes

Pay: £13.00-£14.00 per hour
Expected hours: 15.0 – 25.0 per week
