About the Role
Are you a compassionate, motivated and experienced mental health nurse looking for your next opportunity within community services?
We is looking for an enthusiastic Community Psychiatric Nurse (CPN) to join our dedicated and supportive Community Mental Health Team.
As a CPN, you will play a key role in providing high-quality, person-centred mental health care to adults within the community. You will work collaboratively with service users, carers and a range of multidisciplinary professionals to promote recovery, independence and wellbeing.
You will manage a caseload of service users with a range of complex mental health needs, undertaking comprehensive assessments, developing and reviewing care plans, managing risk and coordinating care in partnership with other professionals and agencies.
Main Duties and Responsibilities
- Provide high-quality, evidence-based nursing care to service users within community settings.
- Manage an allocated caseload and act as care coordinator where appropriate.
- Undertake comprehensive mental health assessments and formulate individualised care plans.
- Monitor and review service users' mental health and respond appropriately to changes in presentation or risk.
- Undertake risk assessments and develop appropriate risk management plans.
- Work closely with psychiatrists, psychologists, social workers, occupational therapists, support workers, GPs and other professionals.
- Support service users in developing recovery-focused goals and promoting independence.
- Administer and monitor medication in accordance with Trust policies and professional standards.
- Provide advice, education and support to service users, carers and families.
- Maintain accurate, timely and confidential clinical records.
- Participate in clinical supervision, team meetings and service development.
- Promote safeguarding and ensure concerns are escalated appropriately.
- Contribute to maintaining a safe, inclusive and positive experience for service users and colleagues.
About You
We are looking for a registered mental health nurse who is committed to delivering compassionate, recovery-focused care.
You will ideally have:
- Current NMC registration as a Mental Health Nurse.
- Relevant post-registration mental health experience.
- Experience of working within community mental health services or a similar setting.
- Experience of managing a clinical caseload and working within a multidisciplinary team.
- Strong assessment, care planning and risk management skills.
- Excellent communication and interpersonal skills.
- The ability to work independently while also contributing effectively to a wider team.
- A commitment to equality, diversity, safeguarding and person-centred care.
- A full UK driving licence and access to a vehicle for work purposes, where required by the service.
