Senior Carer / Care Coordinator
Location: Havant and surrounding areas
Salary: £14.50 per hour weekdays, with enhanced rates for weekends and Bank Holidays. Double pay on Christmas Day and New Year's Day.

About the Role
Serendip Home Care is expanding and is seeking an experienced Senior Carer / Care Coordinator to support the development of our new Havant branch.
This is an excellent opportunity for an experienced care professional looking to progress their career by combining high-quality care delivery with care coordination responsibilities. You will play a key role in ensuring our clients receive safe, effective and person-centred care while supporting the day-to-day operation of the branch.

Key ResponsibilitiesCare Delivery
- Deliver high-quality, person-centred care that promotes dignity, independence and choice.
- Support clients with personal care, nutrition, hydration and medication.
- Monitor clients' health and wellbeing, reporting any changes promptly.
- Build positive relationships with clients, families and colleagues.
- Maintain accurate electronic care records in accordance with company policies and CQC requirements.
Care Coordination
- Complete initial and ongoing care assessments.
- Develop, review and update person-centred care plans.
- Update client tasks, risks and care instructions to reflect changing needs.
- Liaise with GPs, District Nurses, community healthcare professionals and families.
- Support the safe introduction of new care packages.
- Ensure all care documentation is accurate, compliant and up to date.
- Work closely with the Registered Manager to maintain high standards of care.
- Support and guide care staff with care-related queries.
- Assist with quality assurance, audits and continuous service improvement.
About You
We are looking for an organised, compassionate and motivated individual committed to delivering outstanding care.
Essential Requirements
- Previous experience working as a Senior Carer, Team Leader or similar role.
- Experience completing care assessments and developing person-centred care plans.
- Excellent communication, organisational and time management skills.
- Good IT skills and confidence using digital care management systems.
- Ability to communicate effectively with healthcare professionals, clients and their families.
- Good understanding of person-centred care, safeguarding and CQC standards.
- A full UK driving licence and access to your own vehicle (essential).
- Willingness to travel throughout Havant and the surrounding areas.
- The right to work in the UK.
Desirable
- NVQ Level 3 (or equivalent) in Health and Social Care.
- Previous experience in a care coordination or supervisory role.
