TITLE: Care Co-Ordinator
HOURS: 37.5 hours per week over 5 days.
Contract: Fixed-term, 12 months – maternity cover
SALARY: Band 4 (starting at £29,242WTE)

About the Role
Kingston PCN is looking for a caring, compassionate and highly organised Care Co-ordinator to join our team on a 12-month fixed-term maternity cover contract.
This is an exciting opportunity for an individual who enjoys working with people and is committed to helping patients access the right support at the right time. You will work closely with GPs, primary care professionals, the wider multidisciplinary team (MDT), patients, carers and partner organisations to support the delivery of personalised, coordinated care.
General Practice is busy, varied and constantly evolving. The successful candidate will therefore need to be adaptable, able to learn quickly and confident responding to changing priorities. You will be expected to organise and prioritise your own workload, work independently and contribute positively as part of a committed multidisciplinary team.
Key Responsibilities
As Care Co-ordinator, your responsibilities will include, but are not limited to:
Working with GPs and other primary care professionals within the PCN to identify and manage a caseload of patients.
Working closely with the Lifestyle Team, including Social Prescribing Link Workers, Health and Wellbeing Coaches, Dietitians and GP Assistants.
Supporting patients to use decision aids and develop personalised care and support plans in line with best practice.
Supporting the PCN team in the delivery of Network Contract DES specifications.
Helping patients manage their needs by responding to queries and making and managing appointments.
Providing care coordination and navigation, using digital tools where appropriate, across health and social care services.
Supporting the coordination and delivery of MDT meetings across the PCN and its member practices.
Scanning and filing incoming post and electronic clinical correspondence.
Managing incoming email queries from hospitals, care agencies and other healthcare providers.
Managing administrative requests through the practice triage system, including medical letters and report requests.
Signposting patients to appropriate community and local services, working alongside Social Prescribing colleagues.
Acting on communications from hospitals, community providers and other agencies.
Following the appropriate death protocol to support a smooth and sensitive process for families and close relatives.
Reviewing patients with a new cancer diagnosis and liaising with the in-house Social Prescriber to support cancer care reviews.
Managing incoming medical reports and Subject Access Requests (SARs) from patients and third parties.
Supporting and encouraging patients to participate in local and national screening programmes.
Promoting health education and self-care initiatives within the patient population.
Proactively supporting patients to participate in vaccination programmes, including flu, shingles and childhood immunisations.
Organising and prioritising your own workload effectively.
Monitoring outstanding tasks and ensuring they are completed through regular review and audit of the clinical system.
Liaising directly with multiple agencies to coordinate care and support for patients.
Supporting patients and carers and gathering feedback about their experiences.
Collaborative Working
The post-holder will:
Work as part of the Primary Care team and contribute to service evaluation, research and quality improvement activity.
Collaborate effectively with MDT colleagues, patients and carers when coordinating care.
Develop positive working relationships with professionals across health, social care and voluntary/community services.
Use appropriate healthcare technologies to optimise service delivery, patient access and continuity of care.
Communicate effectively with a wide range of professionals, patients and carers.

Education, Learning and Development
The post-holder will be expected to:

Participate in teaching and training of practice and PCN staff where appropriate.

Attend relevant courses, study days and training programmes required for the role.
Undertake additional training where necessary to support enhanced services and PCN/practice initiatives.

Under the supervision and support of the PCN and member practices, the post-holder will:
Participate in clinical governance and quality improvement activities.
Contribute to improving health outcomes through audit, risk management and quality improvement.
Raise concerns relating to risk, quality and patient safety with appropriate colleagues.
Participate in the investigation of incidents and events where required.
Identify and share relevant research findings relating to their area of practice.
Collect and review data for audit purposes.
Reflect on personal and team performance and suggest ways to improve services.
Work effectively with individuals and organisations across different agencies to meet patients' needs.
Manage time, workload and resources effectively.
Meet agreed timescales and deadlines for audits, reporting and other required returns.
Safeguarding Adults and Children
The post-holder has a duty to safeguard and promote the welfare of vulnerable adults and children.

Person Specification
Essential
Experience of using EMIS.
Good working knowledge of Microsoft Office, including Excel.

Additional Information
This is a fixed-term 12-month maternity cover position working 37.5 hours per week, based at Fairhill Medical Practice and supporting the work of Kingston PCN.
