Liberty Private Care is looking for a Care Coordinator to assist in the day to day running of our domiciliary care business. Office hours are 09.00 - 17.00 Monday - Friday with a 30 minute lunch break.
Every day in the office is different but will include communicating with staff, clients and health professionals, taking enquiries from prospective clients, keeping staff and client files up to date and compliant for CQC, processing job applicants, completing the weekly rota and generally being a super hero! Experience with Nourish would be an advantage.
On Call is included one weekend in three 5pm Friday - 9am Monday plus 4 evenings in every 3 weeks from 5pm - 9am , this involves taking emergency out of hours phone calls from staff or clients and if a care visit can’t be covered by regular staff, you would need to go and cover it. A work phone and tablet/ipad are provided for the on call. Our on call is not busy.
Liberty Private Care are looking for someone who is reliable, committed, an excellent communicator, flexible, computer literate and has previous experience as a care coordinator in a domiciliary care setting. You must be a minimum of NVQ 3 qualified in Adult Health & Social Care and a car driver with use of a car in case there is a need to cover a care visit either during office hours or whilst on call. Additional pay will be given for care visits undertaken during the on call period or outside of your working hours at £14.50/hour. Experience with complex care would be an advantage.

Liberty Private Care provides domiciliary care to private clients with care starting at 1 hours care, we provide ‘Live In’s’ and will be starting providing Complex Care this year. Most of our carers are NVQ qualified and all have a minimum of 2 years care experience. We provide a superior level of care to our clients than the norm and have an advanced registration with CQC that most domiciliary care companies don’t have that enables us to deliver more clinical care (TDDI).
