A new opportunity has arisen to join the Neurology and Stroke service as an Outpatient Pathway Coordinator, supporting the smooth and timely progression of patients through their outpatient pathways. This role requires strong organisational and communication skills, alongside close collaboration with a wide range of clinical and administrative teams across St Peter’s, Ashford and Woking Hospitals.
The successful candidate will play a key part in monitoring and validating Referral to Treatment (RTT) 18‑week pathways, ensuring patients are seen within national standards and in line with Trust access policies. Acting as a local RTT specialist , they will help prevent patients being lost to follow‑up, oversee clinic outcome processes, and support performance management across the division.
The ideal candidate will have a good understanding of NHS access standards, be confident using hospital systems, and able to use initiative to identify and escalate pathway concerns promptly. Full training will be provided on Trust systems and processes

As an Outpatient Pathway Coordinator, you will support the smooth and timely progression of Neurology and Stroke patients through their outpatient pathways. Your key responsibilities will include:
Monitoring RTT pathways to ensure patients are seen and treated within national 18‑week standards, escalating delays when required.
Tracking and preventing lost to follow‑ups , ensuring all follow‑up appointments are identified, booked and monitored, and that no patient is lost in the system.
Validating patient pathways from referral through diagnostics and treatment, ensuring accurate and timely data entry on systems such as Cerner, Luna and Referral Tracker.
Liaising with clinical and administrative teams across St Peter’s, Ashford and Woking to coordinate investigations, appointments and pathway milestones.
Managing incomplete clinic outcomes and ensuring all outcomes are recorded and actioned appropriately.
Supporting RTT performance management by identifying pathway issues, highlighting potential breaches and contributing to service improvement work.
Providing guidance and support to colleagues on RTT rules, access policies and pathway processes.
Maintaining effective communication with internal teams and external partners, including GPs and other Trusts, to ensure timely progression of care.

The Outpatient Pathway Coordinator will play a vital role in supporting the Neurology and Stroke service by ensuring patients move smoothly and safely through their outpatient pathways. Working across St Peter’s, Ashford and Woking Hospitals, the postholder will act as a local expert in RTT processes and provide essential support to clinical and operational teams.
You will be responsible for monitoring and validating Referral to Treatment (RTT) 18‑week pathways , ensuring patients are seen within national standards and that any delays or risks of breach are identified and escalated promptly. A key part of the role is preventing patients being lost to follow‑up , tracking all follow‑up appointments, and ensuring clinic outcomes are completed accurately and in a timely manner.
The postholder will work closely with Service Managers, Assistant Service Managers, Clinical Office Teams, Appointment Centre staff and RTT Access & Performance Facilitators to coordinate all elements of the patient journey. This includes monitoring diagnostic requests, ensuring results are received on time, and maintaining accurate pathway information on systems such as Cerner, Luna and Referral Tracker.
You will support the division’s RTT performance by identifying pathway issues, contributing to service improvement work, and providing guidance and training to administrative colleagues on RTT rules and access policies. Strong communication skills are essential, as the role involves liaising with internal teams and external partners, including GPs and other Trusts, to ensure timely progression of care.
The successful candidate will be expected to use initiative, manage competing priorities, and maintain high standards of accuracy to support the delivery of safe, efficient and patient‑centred services.
