Job Overview
We are seeking a dedicated and compassionate Care Coordinator to join our team. The successful candidate will play a vital role in organising and supervising care services within care homes specialising in senior care. This role involves coordinating personalised care plans, supervising staff, and ensuring the highest standards of care are maintained. The ideal applicant will possess strong leadership and communication skills, with experience in supervising teams within a care environment. A valid driving licence and IT proficiency are essential for this position.
Responsibilities
- Organise and oversee personalised care plans for residents, ensuring individual needs are met effectively
- Supervise and support care staff, fostering a positive and professional working environment
- Coordinate daily care activities, ensuring smooth operations within the care home
- Liaise with families, healthcare professionals, and external agencies to facilitate comprehensive care delivery
- Monitor compliance with health and safety regulations and maintain accurate documentation
- Provide leadership during shifts, offering guidance and support to team members
- Utilise IT systems to update records, manage schedules, and communicate effectively across teams
- Drive between different care homes or community locations as required to oversee operations
Requirements
- Proven experience in senior care or a related field, with supervisory or leadership responsibilities
- Strong communication skills, capable of liaising effectively with residents, families, and team members
- Valid driving licence and the ability to travel between multiple locations as needed
- Experience in developing and managing care plans tailored to individual needs
- Proficiency in IT systems relevant to healthcare management
- Supervising experience within a care home setting is highly desirable
- Compassionate approach with excellent organisational skills
- Ability to work independently whilst maintaining team cohesion
Job Type: Full-time
Pay: £26,000.00-£28,000.00 per year
