Torbay Council is looking to appoint Social Care Assessors.
Do you have the skills and experience to help adults live independently, make informed choices, and access the support they need to live good lives?
We are looking for Social Care Assessors to join our Adult Social Care teams. Current opportunities exist within our Working Age Adults Mental Health Service, and our front-end service and we expect further opportunities to become available across Adult Social Care during this recruitment campaign. Suitable candidates may therefore be considered for vacancies arising in a range of teams across the service.
In this rewarding role, you will work directly with adults, their families and carers to understand what matters most to them and identify the support they need to remain safe, well and independent.
You will carry out assessments, develop and review support plans, and work closely with colleagues and partner organisations to ensure people receive the right support at the right time. You will also provide advice and guidance, helping people access local services, community resources and practical support that can make a real difference to their lives.
Working with people who may have complex health, social care or mental health needs, you will help them navigate challenges, achieve their goals and remain as independent as possible. You will need to be confident managing a varied caseload, gathering information, making recommendations and maintaining clear and accurate records.
We are looking for people who are passionate about helping others and making a difference in their community. Whether you already work in health or social care or are considering a career in Adult Social Care for the first time, we want to hear from you.
You will have strong communication and organisational skills, enjoy working with people, and be supported through training and development to succeed in the role.

In Torbay, we are entering an important and positive period of change for Adult Social Care. As part of the planned conclusion of the Section 75 partnership agreement, services currently delivered by South Devon and Torbay NHS Foundation Trust will transfer back to Torbay Council in April next year.
As we move towards this transition, all new and existing social care roles will be employed by Torbay Council. From appointment, colleagues will be part of the Council workforce, benefiting from council terms and conditions, access to the Local Government Pension Scheme, and a range of employment and wellbeing support services.
To ensure continuity of high-quality care for people who use our services, staff will initially work within South Devon and Torbay NHS Foundation Trust. This arrangement will remain in place until the full transfer of services back to the Council at the end of March next year.
This post meets the requirements for an Enhanced Criminal Record Check. This will show spent and unspent convictions and adult cautions which have not been filtered in line with legislation, plus any information held by local Police that is considered relevant to the role.
