We are seeking a motivated and professional Support Worker to join our homelessness accommodation service. This is a rewarding but demanding role supporting individuals to address the issues that have contributed to their homelessness and to successfully move on to independent living.

- Support residents to identify and achieve realistic goals.

- Maintain accurate, timely, and professional records, case notes, and reports.
- Liaise with external agencies, including local authorities, housing providers, healthcare services, and employment support organisations.
- Source suitable move-on accommodation and support residents through the transition to independent living.
- Monitor progress and ensure support plans remain outcome-focused.
- Contribute to the effective operation of the service and work collaboratively as part of a team.
- Promote safeguarding, equality, diversity, and professional boundaries at all times.
Essential Requirements
