Position: Care Management Assistant (BR8) – Social Care Non-Qualified
Location: Churchill Court, 2 Westmoreland Road, Bromley, Kent, BR1 1AS
Start Date: ASAP (target start date: 5 October 2026)
Contract Duration: 12 weeks
Working Hours: 36 hours per week; 3 days remote and 2 days in the office
Pay Rate: PAYE: £18.03 per hour

Job Responsibilities
- Carry out Care Act 2014 assessments and reviews for adults and carers, with appropriate guidance and support.
- Review care and support arrangements for adults living at home, in residential or nursing placements, Extra Care Housing, supported housing, and similar settings.
- Develop and update care and support plans based on assessed needs and agreed outcomes.
- Promote choice and control, including the appropriate use of Direct Payments.
- Monitor services and care packages to ensure they remain effective, appropriate, and proportionate.
- Identify risks arising from changes in care provision and support appropriate safeguarding and risk management actions.
- Undertake Mental Capacity Assessments and contribute to Best Interest decisions where required.
- Provide practical information and support to adults, carers, families, and advocates.
- Work collaboratively with social workers, occupational therapists, health professionals, housing providers, care providers, and other partner organisations.
- Maintain accurate, timely, and defensible case records in line with statutory and organisational requirements.
- Support safeguarding enquiries, duty functions, and other team activities as required.
- Undertake home visits and travel within and, where necessary, outside the borough to visit individuals in their current placements.
- Contribute to service development by sharing relevant information, feedback, and ideas.
- Undertake other duties appropriate to the level of the role.
Person SpecificationMust-Have Requirements
- Experience of working in a care management, health, or adult social care environment.
- Recent experience working directly with adults and carers, preferably including older people or housing-based support.
- Good working knowledge of the Care Act 2014, Mental Capacity Act 2005, and adult safeguarding principles, or a willingness to develop this knowledge.
- Experience of completing assessments, reviews, and/or case management for adults receiving care and support.
- Experience working across a range of agencies and understanding their respective roles.
- Ability to communicate clearly and effectively, both verbally and in writing.
- Ability to work in a strengths-based and person-centred way with adults and carers.
- Ability to manage a caseload, work independently, and respond effectively to changing priorities.
- Ability to work under pressure and manage challenging situations professionally.
- Good IT skills and the ability to maintain accurate electronic records.

- Ability and willingness to undertake home visits and travel across the borough.
- Minimum standard of English equivalent to GCSE Grade A–C.
- Relevant social care qualification such as NVQ is desirable at BR8 but not essential.
- Eligible to work in the UK.
- Satisfactory DBS check required.
Nice-to-Have Requirements
- Experience supporting service transitions, hospital discharges, or changes in care providers.
- Experience working with housing providers or within Extra Care or supported housing services.
- Experience undertaking residential care assessments and preparing panel submissions.
- Experience managing both short-term and long-term assessments or cases.
- Experience working with complex care pathways, safeguarding cases, or higher-risk situations.
- Relevant qualification such as NVQ Level 3 or equivalent.
- Knowledge of local social care services, community resources, and available support.
- Experience supporting or contributing to the induction of new staff.
