We are seeking a dedicated Clinical Pharmacist to join our Pharmacy team on a FlexiBank arrangement at One Ashford Hospital, part of Phoenix Hospital Group. This role offers a unique opportunity to provide pharmaceutical care and support to patients, ensuring the safe and effective use of medications. As a Clinical Pharmacist, you will play a vital role in enhancing patient outcomes and collaborating with healthcare professionals in a dynamic medical environment.
Responsibilities:

Conduct comprehensive medication reviews and assessments for patients.
- Provide expert advice on medication management to healthcare professionals.
- Monitor patient responses to medications and adjust treatment plans as necessary.
- Educate patients on the safe and effective use of their medications.
- Collaborate with multidisciplinary teams to optimise patient care.
- Maintain accurate and up-to-date patient medication records.
- Participate in clinical audits and quality improvement initiatives.
Qualifications:

Bachelor's degree in Pharmacy or equivalent.
- Registered with the General Pharmaceutical Council (GPhC).
- Minimum of 1 year of experience in a clinical pharmacy setting.
- Strong knowledge of pharmacology and medication management.
- Excellent communication and interpersonal skills.
- Ability to work effectively in a team and independently.
- Proficient in using pharmacy management systems and electronic health records.
