Duties
- Coordinate and develop personalised care plans in collaboration with clients, families, and healthcare professionals.
- Act as a primary point of contact for clients, offering guidance and support throughout their care journey.
- Organise appointments, liaise with service providers, and ensure timely delivery of care services.
- Maintain accurate records of client interactions, care plans, and progress reports using IT systems.
- Provide information and advice to clients regarding available services and resources.
- Monitor client wellbeing, addressing concerns promptly and escalating issues when necessary.
- Support training sessions or educational activities related to care provision when required.
Skills
- Excellent communication skills, with the ability to listen actively and convey information clearly in English.
- Strong organisational skills with the ability to manage multiple cases efficiently.
- Proficient IT skills, including experience with data entry, scheduling software, and electronic health records.
- Experience or background in teaching or training is advantageous, enabling effective support and education for clients or team members.

Pay: £28,500.00-£30,000.00 per year
