Job Overview
We are seeking an energetic, compassionate and enthusiastic Lifestyle Community Coordinator to support the wellbeing, independence and quality of life of residents within our care home.
The successful candidate will work closely with the Activities Team, care and nursing teams, residents, relatives, volunteers and local community organisations to create meaningful, inclusive and person-centred experiences.
A key responsibility of the role will be to plan, coordinate and assist residents to access the wider community, enabling them to maintain their interests, independence, relationships and community connections. The role will also provide practical support to the Activities Team with activities, events, celebrations and one-to-one resident engagement.
Duties
- Plan and deliver meaningful, person-centred activities based on residents' interests, abilities and preferences.
- Work closely with and support the Activities Team with daily activities, one-to-one engagement, events and celebrations.
- Plan, coordinate and assist residents with community access, including walks, shopping, cafés, places of worship, social groups and other outings.
- Accompany residents into the community, including lone working with individual residents, where appropriately risk assessed and in accordance with their care plan and the home's lone-working procedures.
- Plan outings, including transport, accessibility, staffing, bookings and appropriate risk assessments.
- Build positive relationships with local schools, community groups, faith organisations, charities, businesses, entertainers and volunteers to create opportunities for residents.
- Organise birthdays, cultural and religious celebrations, entertainment and other special events within the home.
- Encourage residents, relatives and colleagues to participate in activities and community events and obtain feedback to improve the programme.
- Maintain appropriate activity, community-access and resident participation records, including relevant documentation and risk assessments.
- Promote residents' independence, dignity, choice, social inclusion and wellbeing in all activities and outings.
- Follow safeguarding, health and safety, confidentiality, GDPR and other relevant care home policies, reporting any concerns promptly.
- Provide reasonable support to the Activities Team and wider care home as required.
Skills
- Previous experience in activities, wellbeing, community engagement, care, hospitality or a similar people-focused role is desirable.
- A genuine passion for supporting older people and improving their wellbeing, independence and quality of life.
- Experience or understanding of supporting people living with dementia is desirable.
- Strong communication and interpersonal skills with the ability to build positive relationships with residents, relatives, colleagues and community organisations.
- Good organisational and planning skills with the ability to coordinate activities, events and community outings.
- Ability to plan and safely support residents with community access, taking their individual needs, mobility and risk assessments into consideration.
- Ability to work independently and safely on a lone-working basis when accompanying individual residents into the community, following agreed care plans, risk assessments and lone-working procedures.
- Creative and enthusiastic approach to developing meaningful activities and resident experiences.
- Ability to work effectively both independently and as part of the Activities and wider care team.
- Good administrative, documentation and basic IT skills.
- Understanding of person-centred care, dignity, safeguarding, confidentiality and positive risk-taking.
- Flexible, patient and professional approach when supporting residents with different levels of physical and cognitive ability.
Pay: £13.50 per hour
Expected hours: 40.0 per week
