Job Summary
We are seeking a compassionate and dedicated Health Care Assistant/Support Worker to join our team. Responsibilities
- Assist clients with daily activities such as personal hygiene, mobility, and nutrition, ensuring their comfort and dignity.
- Support the implementation of individual care plans tailored to each person's needs, preferences, and health conditions.
- Maintain clear and effective communication with clients, families, and healthcare professionals to ensure coordinated care.
- Use IT systems to document care activities accurately and update records regularly.
- Drive between different locations as needed for home care visits or appointments, ensuring timely support delivery.
- Promote a safe environment by adhering to health and safety guidelines at all times.
- Contribute to team meetings by sharing observations about clients’ well-being and progress.
Qualifications
- Valid driving license with reliable access to a vehicle preferred for home visits.
- Experience working in a care home or providing home care is advantageous but not essential; training can be provided.
- Ability to read, write, and communicate effectively in English.
- Basic IT skills for documentation and communication purposes.
- Strong communication skills with the ability to build rapport with clients and colleagues.
- Knowledge of care plans, health & safety procedures, and safeguarding policies is desirable.
- A caring attitude combined with patience, empathy, and professionalism.
