Overview
We are looking for a committed and experienced Care Manager to support the delivery of high‑quality domiciliary care services, working closely with the Registered Manager. The successful candidate will bring strong leadership abilities, a solid understanding of adult's care needs—including dementia and Alzheimer’s support—and proven experience in managing care plans and medication processes.
This is an excellent opportunity to lead a compassionate team dedicated to promoting independence, dignity, and wellbeing for people receiving care at home. Applicants should have experience in home care or care home management, with supervisory experience highly desirable.
Key Responsibilities
- Lead care teams — Provide supervision, guidance, and support to care staff to ensure personalised, high‑quality care.
- Develop care plans — Create, implement, and regularly review individualised care plans, including specialist dementia and Alzheimer’s support.
- Oversee medication — Ensure safe medication administration in line with legislation and organisational policy.
- Monitor wellbeing — Observe changes in health, liaising with healthcare professionals when required.
- Ensure compliance — Maintain high standards of safety, cleanliness, and regulatory compliance.
- Manage rotas — Oversee staffing schedules, coordinate training, and support ongoing staff development.
- Promote dignity and independence — Foster a positive, respectful environment for all service users.
- Maintain accurate records — Keep clear, compliant documentation relating to care delivery and staff activity.
- Act as a key contact — Liaise with families, healthcare partners, and regulatory bodies.
- Participate in on‑call duties — Take part in an on‑call rota, including alternate weekends, and work in the community when required to cover staff absences or annual leave.
Experience & Skills Required
- Proven experience in domiciliary care or similar care settings
- Strong understanding of adult's care, including dementia and Alzheimer’s support
- Experience developing and managing care plans
- Demonstrated leadership or supervisory experience
- Knowledge of medication administration and compliance standards
- Background in care homes or home care
- Excellent organisational and time‑management skills
- Compassionate, confident communicator with a person‑centred approach
