We are excited to be recruiting a Neighbourhood Care Coordinator.
The Neighbourhood Care Coordinator will act as a trusted, named point of contact for residents with health, wellbeing and social care needs. Working across organisations within the neighbourhood partnership, the post holder will support individuals to navigate services, overcome barriers, and access the right support at the right time.
This innovative role focuses on taking a whole-person approach to care, recognising that a resident’s health, social circumstances and personal goals are often interconnected. The Care Coordinator will work closely with health, social care and voluntary sector partners to ensure coordinated and person-centred support for people living in Buckinghamshire.
The post holder will build strong relationships with residents, families, and professionals across the system. In addition, they will play an important role in improving the experience of residents by navigating them through their health and wellbeing journey, gathering feedback, supporting people to take an active role in their health and wellbeing, and contributing to service development across the neighbourhood partnership.
Act as the named Care Coordinator for a caseload of residents.
Build positive, trusted relationships with residents, their families and carers, supporting them throughout their care journey.
Take a whole-person approach by understanding each resident’s physical health, mental health, social circumstances and personal goals.
Support residents to access services across primary care, community health, mental health, social care, housing and voluntary organisations.
Maintain regular contact with residents to ensure care plans are progressing and any new needs are identified early.
Work closely with wider system partners such as GPs, nurses, social workers, allied health professionals, mental health teams and community organisations to coordinate care and ensure a joined-up approach.
Support residents to understand their conditions, prepare for appointments and increase their confidence in managing their own health and wellbeing.
Collect and record patient feedback and outcome measures to help improve services and resident experience.
Maintain accurate records and manage referrals in line with information governance and data protection requirements.
Coordinate multidisciplinary team meetings and case discussions.
