An exciting opportunity has arisen for an experienced and motivated Senior Commissioning Manager to join the Continuing Healthcare (CHC) Team, supporting individuals with learning disabilities across North West London.
This is a rewarding and influential role within a specialist service dedicated to improving the lives of people with learning disabilities and complex health needs. You will play a key role in ensuring individuals receive personalised, high-quality care that promotes independence, choice, dignity and the best possible health outcomes.
Working at the interface of health, social care and commissioning services, you will coordinate complex care packages, undertake assessments, advocate for individuals and their families, and work collaboratively with a wide range of professionals to ensure care is delivered safely, effectively, and in line with NHS Continuing Healthcare guidance.
This role offers an excellent opportunity for an experienced Registered Learning Disabilities Nurse (RNLD) or healthcare professional who is passionate about reducing health inequalities, improving experiences of care, and influencing service development for people with learning disabilities. Development opportunities may also be considered for the right candidate.
Provide operational leadership, on behalf of the ICB and ICS, to deliver services for specific client groups who required individualised packages of care, to stay well, engage fully in their community, age well and support end of life.
- Participate in the development of specific pathways and commissioning functions, forming collaborative working arrangements which improve the quality and effectiveness of care across the system.
- Ensure the Head of Complex Care has clear line of sight of both the clients requiring individualised care and the associated funding and governance arrangements.
- Ensure best value for public money, improve quality and performance
- Work in partnership with colleagues from the strategic commissioning directorate and borough directorates to develop commissioning plans which support effective individualised commissioning.
- Provide operational leadership to specific borough and function teams within Complex Individualised Care to deliver robust and effective highly complex individualised commissioning processes and systems.
