We are seeking a proactive and compassionate Transition Coordinator to support learners with EHCPs as they transition into, through, and beyond Abingdon & Witney College. Acting as a key link between schools, families, Local Authorities, and the College's SEND team, you will help ensure every learner receives the guidance, support, and opportunities they need to succeed.
Key Responsibilities
- Coordinate personalised transition plans for learners with SEND.
- Build strong relationships with schools, families, Local Authorities, and external agencies.
- Attend reviews, transition meetings, and stakeholder events.
- Support learners in preparing for adulthood, further learning, employment, apprenticeships, or independent living.
- Organise transition events, campus visits, and SEND-focused activities.
About You
- Experience working with young people with SEND.
- Knowledge of SEND legislation and the SEND Code of Practice.
- Strong communication and organisational skills.
- Experience working collaboratively with a range of professionals and stakeholders.
