Job Summary
We are seeking a motivated and patient-focused Clinical Pharmacist to join our friendly and supportive GP Practice team. This is an exciting opportunity to develop your career in Primary Care while delivering high-quality medicines optimisation and patient-centred care.
This is a patient-facing, on-site role, working across our three practice sites. Remote working is not available.
You will work closely with GPs, Pharmacy Technicians, Nurses and other healthcare professionals to improve patient outcomes through safe, effective and evidence-based use of medicines.
This role is suitable for a pharmacist looking to develop or expand their experience in General Practice. Independent prescribing is not essential, and full support will be provided for ongoing professional development within Primary Care.
Main Responsibilities
- Support and undertake patient-facing medication reviews and structured medication reviews (SMRs) where appropriate
- Optimise prescribing for patients with long-term conditions and complex medication needs
- Review hospital discharge summaries and undertake medicines reconciliation
- Respond to medication queries from patients and clinicians in a timely and professional manner
- Support safe and effective repeat prescribing processes
- Identify and reduce medicines-related risks to improve patient safety
- Provide clinical advice to GPs and the wider practice team regarding prescribing
- Support implementation of medicines safety alerts, prescribing guidance and formulary changes
- Participate in prescribing audits and quality improvement initiatives
- Support medicines optimisation work across the practice population
- Maintain accurate and up-to-date patient records using clinical systems
- Work collaboratively with the multidisciplinary team and external healthcare providers
Essential Criteria
- MPharm degree (or equivalent)
- Current registration with the General Pharmaceutical Council (GPhC)
- Experience working in a patient-facing healthcare role
- Strong clinical knowledge and understanding of medicines use
- Excellent communication and interpersonal skills
- Good organisational and IT skills
- Ability to work effectively as part of a multidisciplinary team
- Willingness to work across three GP Practice sites
- Ability to work on-site (non-remote role)
- Availability to work between8:00am and 6:30pm, Monday to Friday
Desirable Criteria
- Experience working in General Practice, Primary Care or PCN setting
- Experience of medicines reconciliation or medication reviews
- Experience using EMIS Web or other GP clinical systems
- Understanding of QOF, prescribing targets and medicines optimisation initiatives
- Willingness to undertake or work towards further Primary Care training
