Do you thrive on helping people navigate change?
At Communitas Clinics, we're on an exciting journey of growth, innovation, and transformation. As we continue to evolve, we're looking for an experienced and passionate Change Manager to help shape how change is introduced, adopted, and embedded across our organisation.
This is more than a project role. It's an opportunity to influence how we work, support our people through change, and help deliver better outcomes for both our colleagues and patients.
Working alongside our Head of Business Transformation, you'll play a pivotal role in ensuring change is not only delivered but embraced.
Why Join Us?
- We believe successful change happens with people, not to people.
- You'll have the opportunity to:
- Lead organisation-wide transformation initiatives
- Influence how change is delivered across services
- Work alongside senior leaders and operational teams
- Create meaningful improvements that impact patient care
- Build a culture of continuous improvement and innovation
- Be part of an ambitious organisation that's committed to doing things better
The Role
As our Change Manager, you'll be the driving force behind the people side of change.
You'll help teams understand what is changing, why it matters, and what they need to do to succeed. Acting as the bridge between project delivery and everyday operations, you'll ensure change is communicated effectively, managed well, and embedded successfully.
From governance and communications to stakeholder engagement and adoption, you'll play a key role throughout the entire change journey.
What You'll Be Doing - Leading Change
- Drive the successful adoption of organisational change across multiple projects and initiatives.
- Assess the impact of change on services, teams, and ways of working.
- Identify risks and develop practical solutions to support successful implementation.
- Monitor organisational readiness and ensure changes become embedded.
Bringing Change to Life Through Communication
- Develop engaging communications that help colleagues understand and embrace change.
- Deliver project launches, go-live communications, leadership briefings, awareness campaigns, and organisational updates.
- Support leaders and managers with key messages and communication plans.
- Share success stories and celebrate achievements.
Managing Change Governance
- Coordinate our CAB-Light change governance process.
- Manage the pipeline of organisational change activity.
- Support prioritisation, decision-making, and governance compliance.
- Maintain clear visibility of change activity across the organisation.
Engaging and Supporting People
- Build strong relationships across departments and services.
- Facilitate workshops, engagement sessions, and stakeholder discussions.
- Support managers in preparing teams for change.
- Listen to feedback and help overcome barriers to adoption.
Supporting Training & Adoption
- Identify training needs arising from change initiatives.
- Develop guidance materials and user resources.
- Work with subject matter experts to deliver effective learning solutions.
- Measure adoption and recommend additional support where needed.
About You
You're someone who genuinely enjoys working with people and helping them through change.
You'll be comfortable balancing multiple priorities, influencing stakeholders, and turning complex information into clear, practical actions.
We're Looking For:
- Experience leading or supporting organisational change initiatives.
- Excellent communication and stakeholder engagement skills.
- The ability to influence and build relationships at all levels.
- Strong planning and organisational skills.
- Experience coordinating governance or change processes.
- Confidence facilitating workshops, meetings, and engagement sessions.
- Experience supporting projects, service improvements, or transformation programmes.
- A proactive, solutions-focused mindset.
It Would Be Great If You Also Have:

- PRINCE2, Agile, MSP or another Project Management qualification.
- Lean Six Sigma experience.
- Healthcare or NHS experience.
- Experience developing internal communications and engagement programmes.
- Knowledge of Microsoft 365, Teams and Planner.
What Success Looks Like
You'll help create an organisation where:
- Change is implemented consistently and effectively.
- Teams feel informed, supported, and engaged.
- New ways of working become embedded and sustainable.
- Continuous improvement becomes part of everyday practice.
