Jurassic Coast Primary Care Network brings together Ammonite Health Partnership, Barton House Medical Practice and Lyme Bay Medical Practice. Our teams work collaboratively across the local area to provide proactive, preventative and coordinated care for patients.
We are now looking for a Digital Care Coordinator to join our friendly PCN Administration Team and support the delivery of administrative, digital and data-led services across our member practices.
Job overview
This is a varied role combining general administration, digital systems, data reporting and service coordination.
The successful candidate will support the day-to-day running of PCN services, help maintain effective administrative systems and use digital tools and population health data to identify patients who may benefit from additional support.
You will also assist with digital healthcare technologies, communications and service improvement, working closely with clinicians, administrators, care coordinators and managers across the PCN.
Responsibilities
Responsibilities will include:
- Supporting general PCN administration, including patient contact, appointment booking, correspondence and inbox management
- Coordinating referrals, waiting lists, caseloads, clinic preparation and follow-up actions
- Maintaining systems such as SystmOne searches, templates, rotas and appointment structures
- Running reports and searches to identify patients who may benefit from PCN services
- Maintaining accurate datasets and supporting data quality
- Updating the PCN website, social media and digital communications
- Supporting colleagues and patients to use digital healthcare tools and systems
- Troubleshooting basic digital or system issues and escalating where required
- Contributing to service development and improvement
Experience
We are looking for someone who:
- Has experience working with healthcare professionals or within the NHS, social care or a related setting (not essential)
- Has experience using digital systems or databases to record, manage or extract information
- Is confident using technology and supporting others with digital systems
- Has strong organisational, communication and teamworking skills
- Is competent in Microsoft Office, including Word, Excel, Outlook and PowerPoint
- Has knowledge of SystmOne or a similar clinical system (not essential)
- Can manage competing priorities and work independently
- Holds GCSE English and Maths, or equivalent qualifications
Experience within general practice, population health reporting, SystmOne reporting, website updates or social media would be an advantage.
