As a Community Care Worker (Social Care Assessor), you will play a key role in supporting adults through their recovery journey, whether they are being discharged directly from hospital or receiving rehabilitation within our Intermediate Care Bed-Based Services. Working alongside Social Workers, therapists, nurses, Trusted Assessors and wider multidisciplinary colleagues, you will undertake strengths-based Care Act assessments, identify eligible care and support needs, coordinate care and contribute to timely discharge planning that promotes independence and prevents unnecessary long-term care. Adopting the principles of the Hospital Discharge Model and Home First, you will support safe, timely and person-centred transitions from hospital and intermediate care into the community, ensuring people receive the right care, in the right place, at the right time.
The role offers a unique opportunity to follow an individual's journey from acute hospital admission through rehabilitation and recovery, helping people regain confidence, maximise independence and achieve the best possible outcomes. Working within both hospital and intermediate care environments, you will build strong relationships with individuals, carers and professionals across health, social care and voluntary organisations to ensure coordinated, seamless care.
This role is primarily based across the Transfer of Care Hub and Intermediate Care Bed-Based Services, with opportunities for hybrid working where service needs allow.

Being at home, or in the most appropriate community setting, provides the best environment for recovery once acute medical treatment has been completed. Remaining in hospital or intermediate care longer than necessary can reduce independence, confidence and overall wellbeing.
Your role will be to complete strengths-based assessments, coordinate care and support planning, and work collaboratively with multidisciplinary colleagues to enable people to move safely through their recovery journey.
You will support people who may require:
- Short-term rehabilitation or reablement to maximise recovery and regain independence.
- Care and support at home, including domiciliary care, equipment and adaptations.
- Ongoing residential or nursing care where longer-term support is required.
- Review of existing care and support arrangements following a period of illness or rehabilitation.
A safe, effective and timely transfer through hospital, intermediate care and back into the community is fundamental to achieving positive outcomes for individuals while ensuring the wider health and care system functions effectively.
