This is an important, hands-on operational role within a 7-day home care service.
It is predominantly Monday to Friday, but it is not a Monday-to-Friday, office-only position.
Before applying, please make sure you are comfortable with the following essential requirements:
- Providing on-call cover every Thursday from 4pm
- Providing on-call cover every 4th Wednesday from 4pm
- Providing on-call cover every 4th Saturday and Sunday from 3pm
- Being flexible to swap or cover occasional on-call periods for colleagues' annual leave
- Working approximately 6 Saturdays and 6 Sundays per year to provide planned office holiday cover, with a corresponding weekday off
- Remaining willing and able to provide hands-on care and personally cover a care visit when this is necessary to maintain a client's care
- Occasionally undertaking client introductions, support visits or spot checks outside normal office hours when required, with time off in lieu where applicable
- Being able to travel promptly within our service area when operationally necessary
These are core responsibilities of the Care Coordinator position.

We are looking for an experienced care professional to join our strong close-knit office team as a Care Coordinator.
The Role
This is a varied and rewarding role within a team of three Care Coordinators working alongside our Operations Manager.
Our Care Coordinators don't simply manage rotas.
They support clients, Community Care Workers and colleagues; respond to changing situations; communicate important information; solve problems; maintain high standards and, when circumstances require it, step back into frontline care themselves.
We are therefore looking for someone who wants greater responsibility within care – not someone looking to move away from care altogether.
You will share responsibility across a range of functions, including:
- Care coordination and rota management
- Client care management
- Supporting our Community Care Workers
- Recruitment and HR administration
- On-call responsibilities
- Quality monitoring
- Accurate recording and communication
- Occasional frontline care delivery
Working Hours
The position is normally five days per week, predominantly Monday to Friday.
Typical office hours are 8:00am–4:00pm, although flexibility is required according to the needs of the service.
Planned Weekend Office Cover
Flexicare at Home operates seven days a week.
Care Coordinators therefore provide planned holiday cover for our regular weekend office team members.
You will normally work approximately:
- 6 Saturdays per year
- 6 Sundays per year
When working one of these weekend days, you will normally take a corresponding day off during the working week.
Weekend office cover is a required part of the role.
On-Call Commitment
Our clients receive care seven days a week, and our Community Care Workers need access to appropriate support outside normal office hours.
Your normal on-call commitment will include:
- Every Thursday from 4pm
- Every fourth Wednesday from 4pm
- Every fourth Saturday from 3pm
- Every fourth Sunday from 3pm
The team supports one another, so you will also need to be flexible about swapping or covering occasional on-call periods to accommodate annual leave and other agreed absences.

What does being on-call mean?
When you are the Coordinator on-call, you take responsibility for issues arising during your period of duty.
These may include:
- Supporting Community Care Workers
- Responding to client or family concerns
- Managing unexpected absence
- Resolving urgent rota issues
- Responding to changes in a client's needs
- Arranging emergency cover
- Personally covering an essential care visit when necessary
We are fortunate to have a highly reliable care team, so emergency care cover is not an everyday occurrence. We also have a robust electronic reporting system for reporting concerns that can wait until the next office working day.
However, unexpected absence can happen in home care. When other appropriate options have been exhausted, the Care Coordinator may need to personally provide the care visit.
We believe strongly in supporting our Community Care Workers and their work-life balance. We do not routinely expect an off-duty Community Care Worker to sacrifice their planned day off simply because the Coordinator on duty would prefer not to provide the visit themselves.
Being willing to remain hands-on and provide care when genuinely needed is therefore an essential requirement of this position.

There may occasionally be a need to adjust your working hours, remain later than originally planned, undertake an introduction, complete a support visit or spot check, or respond personally to an operational issue.
Where appropriate, additional time worked can be balanced throughtime off in lieu in accordance with our arrangements.
Communication and Accountability
Excellent written and verbal communication is essential for this position.
Home care operates continuously, so another member of the team may need to pick up a situation when you are no longer working.
You will therefore be expected to:
- Record information accurately and promptly
- Follow our agreed communication and escalation procedures
- Document client and staff concerns appropriately
- Record actions and outcomes clearly
- Provide effective handovers
- Ensure important information reaches the correct people
- Use our care management and office systems consistently
- Follow agreed policies and procedures
A colleague taking over from you should be able to understand what happened, what you did, what has been communicated and what still needs to happen without having to reconstruct the situation themselves.
Key Responsibilities
Rota & Care Coordination
- Coordinate and manage care rotas to maintain safe and reliable care
- Match Community Care Workers with clients according to needs, skills, preferences and continuity
- Respond effectively to changing care requirements and staff availability
- Manage unexpected absence and rota disruption
- Communicate rota changes accurately and promptly
- Help ensure clients receive their planned care within their contracted time slots
Client Care Management
- Support the day-to-day management of client care
- Maintain accurate and up-to-date records in line with regulatory requirements
- Support and contribute to care plan reviews
- Assist with new client assessments and the setup of new care packages
- Respond appropriately to concerns and changes in clients' needs
- Coordinate and communicate medication changes accurately
- Liaise with healthcare professionals and other external organisations
- Help ensure care remains person-centred and outcome-focused
- Participate in specialist care sign-off where appropriate
- Undertake occasional care support visits and spot checks
- Introduce Community Care Workers to clients when required
Supporting Our Community Care Workers
Our Community Care Workers are central to everything we do.
You will:
- Provide approachable and professional day-to-day support
- Help resolve care and rota concerns
- Ensure workers have the information they need to provide safe care
- Respond appropriately when someone needs support
- Promote good communication between the office and community teams
- Help create a culture in which our employees feel genuinely supported and valued
Recruitment & HR Support
- Support recruitment, screening and interviewing
- Coordinate onboarding and induction
- Support DBS, right-to-work, training and other compliance checks
- Maintain accurate employee records
- Support absence reporting and day-to-day HR queries
- Assist with supervision scheduling, probation reviews and performance support
- Promote employee engagement and retention

Essential Requirements
- Minimum 3 years' experience in home care
- First-hand experience providing care in people's own homes
- Strong understanding of person-centred care
- Ability and willingness to personally provide care visits when required
- Ability to fulfil the stated on-call commitment
- Flexibility to swap/cover on-call periods for colleagues' annual leave when required
- Ability to provide planned Saturday and Sunday office holiday cover
- Ability to start work at 7am when scheduled to cover for colleagues holiday
- Ability to travel promptly to clients within our service area when required
- Strong written and verbal English
- Right to work in the UK
- Enhanced DBS or willingness to obtain one

- NVQ/QCF Level 3 in Health & Social Care or working towards it
- Previous Care Coordinator experience

- Experience within a CQC-regulated home care service

- Teamwork: Your normal finishing time arrives, but an urgent client issue remains unresolved and a colleague is dealing with another priority. How would you approach the situation?
- Communication: A client raises an important concern during your shift. You take initial action, but another Coordinator will need to continue dealing with it tomorrow when you are not working. Briefly explain how you would record and hand over the situation.
- Travel: Are you able to travel promptly within the Wakefield and surrounding service areas to provide an essential care visit when you are on-call if this becomes necessary?
- Right to work: Do you currently have the legal right to work in the UK without requiring visa sponsorship now or in the future?
- IT Skills & Digital Confidence: Flexicare at Home makes extensive use of IT systems to support efficient workflows, care and HR documentation, communication, rota management and operational planning.
This role requires you to confidently use multiple IT systems throughout the working day, including specialist care-management and HR-management software, email and Microsoft Office applications.
