Job Summary

Responsibilities
- Assist patients or residents with personal care tasks such as bathing, dressing, and mobility, ensuring dignity and respect at all times
- Support the development and implementation of personalized care plans tailored to each individual’s needs
- Provide companionship and emotional support to promote mental well-being
- Administer medication as directed by healthcare professionals, maintaining accurate records
- Use IT systems to update patient records, document care activities, and communicate effectively with team members
- Drive between different locations for home care visits, ensuring timely and reliable service delivery
- Communicate clearly with patients, families, and healthcare teams to ensure seamless care coordination
Skills
- Valid driving license with the ability to drive safely between locations for home care assignments
- Experience working in a care home environment or providing home care services
- Ability to understand and follow detailed care plans accurately
- Proficiency in using IT systems for record keeping and communication purposes
- Strong communication skills to effectively interact with patients, families, and colleagues
- Fluency in English to ensure clear understanding of instructions and documentation
- Excellent interpersonal skills with a caring attitude and patience under pressure

Pay: £25,420.00-£35,725.84 per year
