Chronic Disease Nurse
North Glen Medical Practice is seeking an experienced and motivated Nurse with a particular interest in Chronic Disease Management, especially respiratory care, to join our friendly and supportive multidisciplinary team.
This post is for 30 hours per week. Working days and salary are negotiable, and will be dependent on experience (expected to align with NHS Scotland Band 56). Previous experience within General Practice or a community setting is desirable.
We are a patient centred practice serving approximately 8,950 patients, supported by 4 GP Partners and 2 salaried GPs. We use EMIS and Docman as our clinical systems.
Our highly skilled and welcoming team places a strong emphasis on collaboration, peer support and continuous development. The nursing team is led by our Advanced Nurse Practitioner/Nursing Manager, who provides clinical leadership, peer support, and oversight of acute presentations (including paediatrics), as well as supporting home visits.
The wider team includes a prescribing Nurse Practitioner delivering chronic disease management (including diabetes and respiratory care), alongside minor ailment services, two practice nurses, and a Healthcare Assistantall committed to delivering a high standard of patient care.

Person Specification

Experience in Chronic Disease Management (particularly respiratory) is desirable

Experience in primary care or community nursing is preferred

Excellent communication and interpersonal skills, with the ability to educate, support and empower patients

Ability to work autonomously whilst contributing effectively within a multidisciplinary team

Strong organisational and time management skills, with a commitment to high quality, patient-centred care
Eligibility
