We have a great opportunity for a full-time band 6 nurse to work Dementia & Older People’s Mental Health Team based in Worthing West Sussex. The service provides an Intensive Support service for those experiencing crisis operating a 9-5pm 7 days a week including Bank Holidays and a Lead Practitioner role where the post holder manages their own caseload working 9-5 Mon- Friday excluding B/H's.
The successful candidate will join a friendly, highly skilled and enthusiastic multi professional team. We will take a keen interest in helping you to develop the skills important to this role and the post-holder will be encouraged to develop special interests and expertise in line with service and team objectives. You will be part of a highly skilled and supportive multi-disciplinary team. You will receive regular managerial and clinical supervision. This is a challenging but rewarding work as we receive positive feedback from our service users and carers.
The post holder will be expected to travel across the Adur, Arun &Worthing area covered by the team, the ability to travel to remote areas is essential.

Job responsibilities
Clinical
To contribute to the management of a team caseload of clients with complex needs, using CPA principles, evidence based/client centred principles to assess, plan, implement and evaluate interventions, which will be delivered in the Dementia and Older Adults Mental Health Service.
To support/manage a team caseload and attend relevant MDT meetings
To regularly support and cover the duty rota
To provide specialist assessment and intervention as part of a multi-disciplinary approach, with specific responsibilities regarding the care pathway
To provide risk screening and assessment as well as risk management planning
To contribute to and facilitate evidence-based assessments and individual and group interventions
To contribute to the maintenance and development of the Service
To regularly share and supervise students on practice placement, and junior staff as appropriate
