Social Prescribing Link Worker – focus on Long Term Health Conditions and Health inequalities
Hours of work: 18.75 hours
Annual Leave: 30 days plus bank holidays FTE
Salary: Up to £26,608 FTE

Based: Sittingbourne Primary Care Network
Would you like to work for an innovative and progressive charity? Involve Kent aims to improve health and empower people and communities to live well. We are looking for a Social Prescribing Link worker to work with GP surgeries in the Sittingbourne area to improve the wellbeing of their patients. Taking a holistic approach, you will connect people to community groups and statutory services to provide them with practical and emotional support.
You will need a positive ‘can-do’ attitude, high levels of resilience, excellent communication skills and the ability to motivate, empower and support people to achieve their goals.
Please note, you must be able to drive and have access to a vehicle for this role.
This role also includes providing regulated activity with adults and employment is therefore subject to relevant checks with Disclosure and Barring service.
