Job summary
We are looking for a motivated and compassionate Care Coordinator to join our friendly multidisciplinary team.
The successful candidate will support patients with complex or long-term needs by helping them access the right services, coordinating their care, and ensuring they receive a joined-up approach to treatment and support.
You will work closely with GPs, nurses, social prescribers, community services and other healthcare professionals to improve patient outcomes and experience.
Main duties of the role
- Proactively identify and support patients who may benefit from coordinated care.
- Develop and maintain personalised care and support plans.
- Act as a point of contact for patients and carers with ongoing healthcare needs.
- Support patients to understand and navigate health and care services.
- Coordinate appointments, referrals and communication between services.
- Work closely with clinicians to ensure patients receive appropriate follow-up and support.
- Assist with case management of patients with complex needs, frailty and long-term conditions.
- Identify and address barriers to accessing healthcare services.
- Support population health initiatives and quality improvement projects.
- Maintain accurate patient records and document all interventions appropriately.
- Attend and contribute to multidisciplinary team meetings.

Person specification
Essential
- Experience working within healthcare, social care or a patient-facing support role.
- Excellent communication and interpersonal skills.
- Ability to build positive relationships with patients and professionals.
- Strong organisational and administrative skills.
- Good IT skills, including Microsoft Office and clinical systems.
- Ability to work independently and as part of a team.
- Understanding of confidentiality and information governance requirements.
Desirable
- Experience working in General Practice or Primary Care.
- Knowledge of EMIS Web.
- Experience supporting patients with long-term conditions, frailty or mental health needs.
- Understanding of personalised care and care planning.
- NVQ Level 3 or equivalent qualification in health, social care or related field.
