Job Description - Care Coordinator:
Responsibilities: As Care Coordinator you are the first line manager for your colleagues and an advocate for the clients. This means helping to ensure that all calls ae adequately covered and that any issues that clients may raise are dealt with without delay. As a care coordinator you are required by the company to be involved in the following:
- Direct observations &spot checks, conducting client assessments & care reviews,
- On Call Cover,
- maintain staff morale,
- attending management meetings,
- regular reviewing of the performance of senior/supervisory staff to ensure that all of the required standards are met and upheld to a high level of performance.
JOB SPECIFICATION:
Essential requirements:
- Experience as a Care Coordinator within Health and Social Care
- Experience in care provision at a level that reflects the responsibilities of the post applied for
- Good literacy and numeracy skills
- Good presentation skills
- Computer literate
- Excel and Word competent
- Understanding of legislation concerned with care provision
- Full driving license
Desirable:
- NVQ 2(Minimum)
- Professional qualification
- Experience as a care supervisor or team leader or other role within a care-providing organisation.
- Proficient use of 'PASS System' and 'CarePlanner' platforms
