Jurassic Coast Primary Care Network is looking for an experienced and motivated Advanced Clinical Practitioner to join our developing Ageing Well Team.
This is a varied community-based role supporting older people across West Dorset, including housebound patients, people living with frailty and residents of local care homes. You will work autonomously while being supported by a friendly and experienced multidisciplinary team.
About the role
Your day-to-day responsibilities will include:
- Undertaking acute clinical visits to housebound patients
- Completing planned and responsive care-home ward rounds
- Carrying out comprehensive clinical assessments
- Triaging patients and identifying clinical priorities
- Formulating differential diagnoses and management plans
- Prescribing medication where appropriate
- Recognising and responding to clinical deterioration
- Escalating patients for further assessment or hospital admission when required
- Supporting admission avoidance and continuity of care
- Coordinating care with GP practices, care homes, community services, social care and the wider multidisciplinary team
- Maintaining accurate and timely clinical records
- Contributing to clinical governance, audit, quality improvement and service development
The role involves managing a varied and time-sensitive caseload, so you will need to be confident making autonomous clinical decisions, prioritising clinical risk and responding flexibly to changing demands.
About you
We are looking for someone who:
- Is a registered healthcare professional
- Is a qualified independent non-medical prescriber
- Has completed, or is working at, an appropriate level of advanced clinical practice
- Has strong acute clinical assessment and decision-making skills
- Has experience supporting older people, housebound patients or care-home residents
- Is confident working autonomously and taking responsibility for clinical decisions
- Can recognise deterioration and manage clinical risk appropriately
- Communicates effectively with patients, families, carers and other professionals
- Works well as part of a multidisciplinary team
- Is organised, proactive and able to manage competing priorities
- Is committed to delivering safe, compassionate and person-centred care

Additional information
Travel between patients’ homes, care homes and PCN sites will be required. Applicants must therefore be able to travel independently across the Jurassic Coast PCN area.
Working days and hours will be discussed with the successful applicant and will depend on the individual’s experience, qualifications and the needs of the service.
Pay: £20.00-£30.00 per hour
