Job Overview
We are seeking a dedicated and organised Care Coordinator specialising in Domiciliary Care to join our team. The successful candidate will be responsible for coordinating personalised care plans, supervising care staff, and ensuring the delivery of high-quality care to clients in their own homes. Responsibilities
- Develop, implement, and review personalised care plans tailored to each client’s needs.
- Coordinate and supervise care staff to ensure consistent delivery of high-quality domiciliary care.
- Conduct regular visits to clients’ homes to assess care quality and address any concerns promptly.
- Organise staff rotas, ensuring adequate coverage for all scheduled visits.
- Cover care calls at short notice to ensure continuity of care.
- Provide leadership and support to care staff, fostering a positive team environment.
- Maintain accurate records using IT systems, including care documentation, scheduling software and follow company policies.
- Ensure compliance with all relevant health and safety regulations and standards.
- Required to take part in an on-call rota to provide out-of-hours support, including evenings, weekends, and bank holidays.
- Drive between various client locations as required, maintaining a valid UK driving licence.
Experience
- Minimum of 1-2 years experience working as a Care Coordinator is highly desirable.
- NVQ Level 3 in Health and Social Care (or equivalent) or relevant experience in a similar role.
- Supervising or leadership experience within a healthcare environment is essential.
- Proven ability to develop and manage Care Plans effectively.
- Strong communication skills for liaising with clients, families, and third parties.
- Proficiency in IT systems for documentation, scheduling, and reporting purposes.
- Full UK driving licence with access to your own vehicle for travel between client locations is essential.
Our head office is located in Wolverhampton, and we provide services across the surrounding areas, including Cannock, Stafford, and Newcastle-under-Lyme.
Hours:
37.5 hours per week
