Job Summary
We are seeking a dedicated and compassionate Patient Care Navigator to join our team. The successful candidate will play a vital role in guiding patients through their healthcare journey, ensuring they receive appropriate support and information. Duties
- Act as the primary point of contact for patients, offering guidance and support throughout their healthcare experience
- Assist patients in scheduling appointments, follow-ups, and coordinating care plans
- Provide information about treatment options, hospital procedures, and available resources
- Liaise with medical staff to ensure seamless communication between patients and healthcare providers
- Maintain accurate and confidential patient records using office software systems
- Follow up with patients post-appointment to address concerns or questions and ensure ongoing support
- Organise and manage administrative tasks related to patient care, including documentation and data entry
Skills
- Previous office experience is highly desirable, demonstrating organisational proficiency and familiarity with administrative procedures
- Excellent communication skills, with the ability to empathise and connect effectively with diverse patient populations
- Strong organisational skills with attention to detail to manage multiple tasks efficiently
- Ability to work collaboratively within a multidisciplinary team
- Compassionate attitude with a commitment to delivering exceptional patient-centred care
- Proficiency in office software such as Microsoft Office or equivalent programmes
