Advanced Nurse Practitioner / Advanced Clinical Practitioner (ANP/ACP) – Home Visiting Service
Vela Medical Group is looking for experienced Advanced Nurse Practitioners (ANPs) and Advanced Clinical Practitioners (ACPs) to join our growing primary care home visiting service across East Sussex and West Sussex.
We provide high-quality clinical care to patients in their own homes, supporting the delivery of urgent and primary care outside of standard GP opening hours.

East Sussex & West Sussex
Home visiting opportunities may be available across a range of locations within the region, making this an excellent opportunity for clinicians who are looking for flexible local work.
The Role
We are seeking confident, autonomous clinicians who are comfortable assessing and managing a broad range of primary care presentations in a community setting.
Sessions are available out of hours, including:
- Weekday evening sessions
- Overnight sessions
- 24-hour weekend service
- Flexible sessional working to suit your availability
This is an excellent opportunity for ANPs/ACPs looking for flexible out-of-hours work, alongside other clinical commitments or as part of a regular portfolio.
Key Responsibilities
- Conduct comprehensive clinical assessments in patients' homes
- Diagnose and manage a wide range of acute and primary care presentations
- Develop appropriate treatment and management plans
- Prescribe safely within your scope of practice
- Recognise and appropriately escalate patients requiring further assessment or emergency care
- Maintain accurate and contemporaneous clinical records
- Work autonomously while having access to appropriate clinical support
- Deliver safe, patient-centred care in a community setting
About You
You will be an experienced ANP or ACP with a strong background in primary care, urgent care, emergency care, or a similar clinical environment.
You should have:
- Current NMC registration (for ANPs) or relevant professional registration
- Appropriate Advanced Practice/ACP or ANP qualification
- Significant experience assessing and managing patients autonomously
- Independent prescribing qualification
- Excellent clinical assessment and decision-making skills
- The ability to recognise deteriorating or seriously unwell patients
- Excellent communication and interpersonal skills
- A professional, reliable and patient-focused approach
- The ability to work independently while visiting patients in their homes
A full UK driving licence and access to a suitable vehicle are required for home visiting.
