- Work within our innovative Anima digital triage team, supporting timely access to primary care.
- Provide advanced clinical care within our established care home service, working closely alongside GP colleagues to deliver proactive ward rounds, comprehensive care planning and high-quality patient-centred care.
- Undertake face-to-face clinical sessions within our GP practices.
- Contribute to our growing Primary Care Research Team, supporting the development of research within general practice and helping shape the future of patient care.

About you:
We are looking for a compassionate, reliable and proactive Advanced Clinical Practitioner who is passionate about delivering outstanding patient care and improving health outcomes.
You Will:
- Be a registered Advanced Clinical Practitioner.
- Ideally have experience working within General Practice (although this is not essential).
- Be confident working autonomously whilst recognising the value of multidisciplinary teamwork.
- Demonstrate excellent clinical assessment, decision-making and communication skills.
- Share our commitment to delivering high-quality, patient-centred care.
The role will include occasional home visits for housebound patients, care home ward rounds and proactive care planning.
