The Assistant Practitioner works as an integral member of the multidisciplinary stroke team, delivering high-quality care and rehabilitation to patients following a stroke.
Working under the supervision of registered nurses and allied health professionals, the post holder undertakes delegated clinical activities, supports rehabilitation programmes, and contributes to the assessment, treatment and ongoing care of patients throughout their stroke recovery journey.
The post holder will promote patient independence, maximise functional recovery, and support patients and their families through education and rehabilitation. The role requires effective communication, a sound understanding of stroke care pathways, and the ability to work across professional boundaries whilst maintaining a patient-centred approach.
The Assistant Practitioner will play a key role in coordinating aspects of the patient's stroke journey, including initiating and facilitating referrals to internal and external services, ensuring timely access to specialist support and community-based resources. The post holder will actively participate in multidisciplinary team (MDT) meetings, contributing relevant patient information, supporting discharge planning, and helping to ensure coordinated, seamless care across the stroke pathway.
Working within agreed competencies and under the supervision of registered healthcare professionals, the post holder will undertake delegated clinical, rehabilitation and care coordination duties.
The post holder will deliver rehabilitation programmes and therapeutic interventions to support patients in achieving rehabilitation goals, promoting recovery and maximising independence. They will assist with mobility, activities of daily living, communication, nutrition and hydration, whilst monitoring and recording patient progress and escalating concerns appropriately.
The Assistant Practitioner will coordinate referrals to internal and external services, liaise with community teams and support discharge planning. They will attend and contribute to MDT meetings, ward rounds and discharge planning discussions, sharing relevant information to support decision-making and continuity of care.
The post holder will provide information and support to patients, relatives and carers throughout the stroke pathway and maintain accurate patient records in line with professional and organisational standards.
Working collaboratively with nursing, medical, therapy and community colleagues, the Assistant Practitioner will contribute to audit, quality improvement and service development activities.
The ability to speak Welsh is desirable for this post; Welsh and/or English speakers are equally welcome to apply.
