About the Role:
A key part of our Adult Social Care Strategy, and as a direct response to resident feedback, our new Help to Live at Home Model was developed to support adults to live independently in their own homes and communities. Providing both short- and longer-term support, including a rapid response service to reduce hospital admissions, the model also provides a night visiting service.
The Senior Support Coordinator will work closely with the Support Coordination Manager and provide support with a range of tasks, becoming a valuable interface between operational social care practitioners and service providers, supporting care planning, the brokerage of services and the accurate recording of service and financial information.
The role will help challenge the status quo and help lead change to ensure Adults within South Tyneside are supported to Live Better Lives.
About You:
You will have significant experience of operational issues within a regulated health and social care environment and be proficient in the use of ICT, including Social Care Databases, such as Liquidlogic. With excellent communication skills you with be able to develop and maintain effective relationships with a wide range of stakeholders.
You will need the confidence and ability to demonstrate sound professional judgements, when making decisions and have the ability to support real change.
