People experiencing homelessness have some of the poorest health outcomes in society. They often develop advanced illness at a much younger age than the general population and face significant barriers to accessing health and social care. Too many people die without their palliative care needs being recognised or addressed.
We are looking for a compassionate, skilled and motivated individual to join our team as a Complex Needs Worker, supporting people experiencing homelessness who have palliative and end-of-life care needs.
In this role you will:

Work with clients with palliative care need in partnership with the lead worker, listening to clients’ values and choices, supporting clients to access palliative care and advocating for their choices to be honoured.

Offer advice and support, both emotional and practical, to staff working with clients with complex and palliative care needs, helping to create a supportive and trauma-informed environment.
- Identify, develop and maintain external relationships with statutory and non-statutory service providers to increase accessibility to support for clients with palliative care needs.

Together with the Palliative Care Manager and Complex Needs Team, develop and deliver training sessions to St Mungo’s staff in palliative care and homelessness and other health-related matters.
About you
The successful candidate will be:

Experienced in supporting people with complex needs, ideally within homelessness, palliative care, mental health, substance use or another related field.

Compassionate, resilient and able to navigate complex situations with professionalism and empathy.

An excellent communicator with strong relationship-building and multidisciplinary working skills.

Flexible, able to work well on your own, know when to seek support and be willing to travel to a range of different services.
