Northfield Medical Centre provides general medical services for approximately 12,300 patients living in Blaby and the surrounding area. We are a teaching and training practice with an experienced multi-disciplinary compliment of clinical staff and a very supportive administrative team.
The successful candidate will ideally have experience in chronic disease management. Anticipated duties and responsibilities are listed in the below job description.
The following are the key tasks of the Practice Nurse. There may be, on occasion, a requirement to carry out other tasks. This will be dependent upon factors such as workload and staffing levels:
· Chronic Disease Management ideally with a specialist interest in COPD/Asthma
· Long Term Conditions management including Diabetes, Heart Disease
· Annual reviews following all QOF requirements
· Health Promotion and screening including cytology, family planning and women’s health
· Recording clinical data such as weight, measurements, temperature, urinalysis and BP
· Wound care / removal of sutures
· Routine Immunisations / Childhood Immunisations
· Hypertension reviews
· ECG’s
· Travel vaccinations and advice
· Request pathology services as necessary & process results as required
· Provide chronic disease clinics, delivering patient care as necessary, referring patients to secondary/specialist care as required
· Home visits as required
· Any other delegated duties appropriate to the post
Education:
- Diploma of Higher Education (required)
Experience:
- Nursing: 3 years (preferred)
Licence/Certification:
- Driving Licence (preferred)
- NMC (required)
