We are seeking a motivated Band 5 Staff Nurse to join our dedicated team on the Transplant Unit at Harefield Hospital. The unit has thirty-four beds across two wards (Rowan and Fir Tree), caring for patients awaiting assessment, those on the waiting list, post-transplant care, and individuals with ventricular assist devices.
The service includes the artificial heart service, recipient transplant co-ordination, and a specialized clinic featuring the UK's first cardiothoracic transplant nurse consultant. Our wards feature extended care facilities and were the first cardiothoracic centre to offer HDU haemo filtration in a ward setting. Nursing care is guided by the Orem model to empower patient independence and understanding. If you are eager to develop your career in a world-renowned specialist environment, we would love to hear from you.
The post holder delivers safe, high-quality, patient-centred care under the guidance of the sister/Charge Nurse and multidisciplinary team (MDT). Key duties include carrying out robust clinical assessments, planning programmes of care, and evaluating interventions without direct supervision.
You will manage the administration of prescribed medications, undertake physical observations, and recognise and escalate clinical deterioration. You will act as a patient advocate, ensuring care respects individual customs and dignity while establishing safe discharge plans.
Additionally, you will support and develop learners and junior staff, act as a mentor or preceptor, and participate in clinical audits, quality improvement, and service development. Accountable for your own nursing practice in line with NMC standards and Trust policies, you will maintain exacting standards of clinical governance, effective MDT communication, and promote a compassionate, safe care environment.
