This is an exciting opportunity to join the Transforming Lives Team. As a valued member of this team, you will need to have a good understanding and experience of the application of the Care Act 2014, Mental Capacity Act 2005 and Mental Health Act 1983. You will be expected to undertake the role of Care Act assessments, support planning, risk assessing, commissioning and planning discharges for people with significant care and support needs under the Learning Disability and Autism Programme.
You will have experience of working creatively with adults who have care and support needs and will bring a commitment to the principles of strength’s-based practice, promoting independence and wellbeing whilst safely managing risks.
You will be working in an exciting and sometimes challenging environment in close partnership with external agencies and providers, which will require high levels of confidence and independence.
You will need to have effective communication, negotiation, organisational and IT skills which are essential tools to aid developing flexible working practices. The team has adapted hybrid working, where you will be partly office and partly home-based working.
In return, you will receive plenty of support and training to help you reach your potential. We will provide regular formal and informal supervision due to the nature of the complex case work.
