At GoodOaks Home Care Wokingham and Bracknell, we're looking for an experienced Care Coordinator to become the heart of our growing office team.
This is much more than a scheduling role. You'll take ownership of our care delivery, ensuring every client receives exceptional, reliable care while supporting our fantastic team of Care Professionals. As we continue to grow, you'll play a key role in delivering over 200 hours of care each week across Wokingham, Bracknell and the surrounding areas.

The Role
You'll take full ownership of our care staff rotas, ensuring every visit is covered safely, efficiently and with continuity for our clients.
Using Access Care Planning (ACP/People Planner), you'll build and maintain recurring visit schedules, monitor live care delivery throughout the day, manage changes, absences and emergency cover, and make sure visit information is accurate for payroll and invoicing.
You'll be the main point of contact for carers, clients, families and healthcare professionals, helping to resolve issues quickly while maintaining the highest standards of customer service.
You'll also work closely with the Registered Manager and wider office team to support compliance, maintain excellent service standards and participate in our shared out-of-hours on-call rota. In the rare event of an emergency, you'll be willing to step in and deliver care visits when required.
About You
We're looking for someone who has:
- Demonstrable experience as a Care Coordinator or scheduling role within care.
- Strong experience managing complex rotas and care schedules.
- Excellent knowledge of Access Care Planning (ACP/People Planner).
- The ability to monitor live care delivery, identify potential issues early and resolve them quickly.
- Outstanding organisational skills and exceptional attention to detail.
- A calm, positive and solutions-focused approach under pressure.
- Excellent communication skills and confidence speaking with carers, clients and healthcare professionals.
- Strong IT skills and a professional telephone manner.
- A full UK driving licence and access to a vehicle.
- A willingness to participate in the on-call rota and provide emergency care visits if needed.
An Enhanced DBS check will be required, which we'll arrange for you.
