The Care Coordinator is the first line of contact for all clients, care workers, nurses and other professionals. They work alongside the Clinical Lead/Registered Manager organizing and managing the day-to-day running of the service provision through the policies, training, supervision and guidance provided by the Company.
Key responsibilities
Duties include:

To manage client relationships.

To understand and match the needs of our clients with suitably qualified care workers/ nurses.
- Supporting the Clinical Lead in achieving the aims and objectives of the company and achieving quality levels.

Assisting in marketing the services provided by the company and liaising with commissioners, prospective Service Users, health professionals and other agencies connected with training and development of the service.

To provide good quality services to clients whilst continually meeting targets and meeting our client’s needs.

To assist in managing and maintaining the administrative systems, ensuring their proper maintenance and avoiding duplication.

To ensure care workers/ nurses are permanently assigned to the clients for continuity of care.

To assist with recruitment, and client service reviews, assessments and responding to emergencies arising.

To communicate with CCG/Social Services Brokers to accept packages of care.

To participate fully as a part of the Operations Team.
Client Management:
-
Key Account Management – act as point of contact for clients and care commissioners.

Check information/client requirements in line with key processes and legislation.

Prepare and send rotas for care workers/ nurses weekly; and ensure that all bookings are filled .

On call support based on a rota.

Identify client needs and provide feedback on success of filling jobs.

Ensure carers are permanently assigned to Service Users.
Recruitment and Client Management:

Manage availability of care workers/nurses to meet client needs

Assist the Clinical Lead in reviewing the progress and performance of care workers/nurses.

Provide information on clients and job specifications to nurses’/care workers
Interviewing candidates

General:
-
To ensure that all assigned work is carried out according to RMS’s policies and procedures.

To undertake any other duties at an appropriate level of responsibility as requested.

In agreement with Clinical Lead, undertake such training necessary to the continuing personal development of skills, knowledge and practice.

Annual Gross Salary: £28000-£32000 ( dependent on experience)
