Care Navigator – Woodhall Farm Medical Centre
Location: Hemel Hempstead
Salary: £12.71 per hour
Hours: Zero hours contract with a possibility of a permanent position
Are you passionate about patient care and excellent customer service? Woodhall Farm Medical Centre is seeking a Care Navigator to join our friendly and supportive team. This role is vital in ensuring patients receive the right care at the right time.
Essential Requirements:
- Full UK Driving Licence.
- Excellent customer care and communication skills.
- Strong command of the English language, both written and spoken.
- IT literacy, with proficiency in basic software such as Microsoft Word and Excel.
- Ability to work effectively as part of a team and provide evidence of teamwork skills.
Desirable Skills & Experience:
- Knowledge of EMIS (clinical system).
- Previous experience working in a GP practice or similar healthcare setting.
