Job Summary

Responsibilities
- Support patients and residents with personal care tasks such as bathing, dressing, and mobility assistance
- Assist with meal preparation and feeding, ensuring nutritional needs are met
- Maintain accurate care plans by documenting observations, progress, and any changes in condition
- Communicate effectively with team members, patients, families, and other healthcare professionals to coordinate care
- Drive or accompany patients to appointments or activities as needed, ensuring safe transportation
- Use IT systems to update records, manage schedules, and access relevant patient information
- Follow health and safety protocols diligently to promote a safe environment for all
Skills
- Strong communication skills in English to interact clearly with patients and colleagues
- Ability to operate IT devices such as tablets or computers for documentation and communication purposes
- Valid driving license and experience driving in various conditions for home care or care home visits
- Knowledge of care plans and the ability to implement them effectively in daily tasks
- Experience working in a care home or providing home care services is highly desirable
- Excellent interpersonal skills with a caring attitude towards individuals of diverse backgrounds
- Good understanding of health and safety regulations relevant to healthcare settings
