Job summary
The Senior Community Mental Health Practitioner will work as a core member of the Community Mental Health Team to co-ordinate the care of a complex caseload of service users, supervising Band 5 Community Practitioners and Band 3 Community Healthcare Support Workers where appropriate.
Working under the supervision of the Deputy Team Manager/Team Manager the practitioner will formulate care and risk management plans in accordance with identified need and ensure that these are recovery focused and developed in collaboration with the service user and where appropriate their relative and carer.
At present, the Trust is unable to offer Skilled Worker visa sponsorship for this post. Applicants must have the right to work in the UK at the time of application.

Main duties, tasks & skills required
To effectively co-ordinate the care of a complex caseload of service users with severe and enduring mental illness.
To undertake initial assessments where this is clinically appropriate.
To formulate care plans in line with identified need.
To provide recovery focussed therapeutic interventions in line with the agreed care plan and risk assessment.
To assess risk, develop risk management plans, review and adjust in line with the service user's needs.
To work in collaboration with service users as experts in their own care, promoting the principles of recovery, choice and personalisation, which value and empower the service user.
To use tools which promote user empowerment e.g. Wellness Recovery Action Plans (WRAP), Recovery Star
To be proactive in ensuring service users receive the range of interventions they need in line with their care plan, whether or not the post holder provides those interventions. This will include physical health monitoring.
To have a proactive approach to the role and needs of carers / families.
To be proactive in monitoring mental health needs and respond in a timely fashion if needs change adjusting the care plan accordingly.
To participate fully in caseload supervision and management, to ensure that care plans are matched to need.
