Job Overview
We are seeking a dedicated and compassionate Care Navigator to join our team. The Care Navigator will play a crucial role in assisting patients and their families in navigating the healthcare system, ensuring they receive the appropriate resources and support. This position requires a strong understanding of office operations, excellent communication skills, and a commitment to providing exceptional care.
Duties
- Maintain and monitor the practice appointment system
- Process personal, telephone and e-requests for appointments
- Answer incoming phone calls, transferring calls or dealing with the callers’ request appropriately
- Process Healthier Together e-requests as they come in
- Signpost patients to the correct service
- Initiating contact with and responding to, requests from patients, team members and external agencies
- Clinically code data on Emis
- Data entry of new and temporary registrations and relevant patient information as required
- Input data into patients’ healthcare records as necessary
- Direct requests for information, i.e., SAR, insurance/solicitors’ letters and DVLA forms to the administrative team
- Manage all queries as necessary in an efficient manner
- Carry out system searches as requested
- Maintain a clean, tidy, effective working area at all times
- Monitor and maintain the reception area and notice boards
- Support all clinical staff with general tasks as requested
- Action incoming emails and correspondence as necessary
- As required support Prescription clerks in the management of repeat prescriptions, ensuring they are processed accurately and efficiently
- Produce, maintain and participate in the receptionist rota for lunch and out of hours cover
We are looking for a candidate to work the following shift patterns:
AM - 07:45 - 13:00
PM - 13:00 - 18:30
All day - 07:45 - 18:30
We are looking for up to three candidates.
Experience
- Previous experience in an office environment is desirable but not essential, preferably within a healthcare setting.
- Strong organisational skills with the ability to manage multiple tasks effectively.
- Excellent verbal and written communication skills, with a focus on patient-centred care.
- A compassionate approach to patient care, demonstrating empathy and understanding in all interactions.
