Job Description:
The post-holder will undertake housing-related support need and risk assessments, deliver short term housing-related support interventions / risk assessments, to be a source of information and advice to colleagues within Housing teams.
To fully understand, promote and contribute to the authority's objectives to prevent homelessness and the contribution Housing Support plays in this in accordance with the Welsh Government's aspiration of homelessness being "rare, brief and unrepeated".
Main Duties:
1. To undertake a full and effective assessment of housing support needs and risk, capturing all the relevant information, from a wide variety of sources including statutory and non-statutory stakeholders, needed to devise and implement individual person-centred support plans with service users, maintaining accurate case notes, in line with the authority's GDPR and confidentiality policies and procedures.
2. To be a point of contact for housing support matters and enquiries within the Authority's Housing Advice and Prevention Team in regards to individuals or households who are homeless, at risk of homelessness or are experiencing financial difficulties.
3. To provide short term / early intervention housing support interventions for service users.
4. To submit support applications for individuals who may have on-going or longer-term support needs to the Newport Supported Housing and Floating Support Gateway.
5. To support project work specific to a commissioned support provider service e.g. where a service is failing, being decommissioned, remodelled or commissioned with an aim to ensuring the need of service users on support are identified so that support can be improved, transfer or ended.
6. To deliver effective signposting to specialist advice and support services as required.
7. To liaise with the Authority's Housing Advice and Prevention Team and work pro-actively to ensure service users are able to access relevant housing services.
8. To maintain and improve knowledge of relevant legislation, regulations and guidance relating to the provision of housing support through professional development. To support service users and colleagues to understand their legal rights and responsibilities under the Renting Homes (Wales) Act 2016, Housing (Wales) Act 2014 together with any other relevant legislation.
9. To work flexibly at locations throughout the city, at the reasonable request of senior Housing Support Team colleagues, including home and community visits (when safe and practicable to do so) to access service users in an effective manner.
10. To co-work effectively with key officers from other statutory agencies/ departments and voluntary sector organisations to achieve positive outcomes for people and to ensure the authority meets its obligations.
11. To compile and maintain a directory of services relevant to the housing support service as a resource tool for colleagues within the authority and other stakeholders.
