We are currently seeking an experienced Service Coordinator to join a community-based social inclusion service in Essex on a 3-month temporary basis.
The successful candidate will play an important role in coordinating support for service users, volunteers and peer mentors, while developing effective relationships with partner organisations and community services.
Key Responsibilities
- Coordinate the delivery of community-based support, groups and activities.
- Engage with new service users and ensure referrals and associated documentation are completed accurately.
- Recruit, support and coordinate volunteers and peer mentors.
- Work closely with partner organisations to identify and support appropriate service users.
- Coordinate appropriate matching of volunteers/peer mentors with service users, taking individual needs and risk into consideration.
- Maintain accurate records, case files, databases and other required documentation.
- Complete and review relevant risk assessments and ensure appropriate safeguarding procedures are followed.
- Develop and maintain effective relationships with community organisations and relevant support services.
- Facilitate groups, activities and sessions where required.
- Support individuals with complex or additional needs and work collaboratively with other agencies.
- Attend multi-agency meetings, case conferences and team meetings as required.
- Support referral pathways and effective communication between services.
- Collect and maintain relevant information for monitoring and reporting purposes.
- Work as part of a wider team and contribute to the development and improvement of the service.
- Maintain professional boundaries and work in accordance with safeguarding, confidentiality, health and safety and equality policies.
- Some flexibility may be required, including occasional evening or weekend work.
- Travel to different community locations across the area will be required.
About You
We are looking for someone who has:
- Experience working within community support, care, mental health, substance misuse, social inclusion or a similar environment.
- Experience of coordinating services, projects, volunteers or peer support programmes.
- Excellent communication and interpersonal skills.
- Strong organisational skills and the ability to manage a busy workload.
- Experience of working collaboratively with external agencies and partners.
- Experience maintaining accurate client records and case management information.
- A good understanding of safeguarding, confidentiality and risk management.
- Strong IT skills, including Microsoft Outlook, Word and Excel, together with experience using databases or case management systems.
- The ability to work independently while also contributing effectively as part of a wider team.
- A professional, empathetic and person-centred approach.
- A flexible approach and willingness to travel between community locations.
Qualifications & Requirements
- S/NVQ Level 3 in Care or equivalent, or relevant experience/qualification.
- Relevant experience within social care, mental health, substance misuse, community support or a comparable setting.
- Valid UK driving licence and access to transport would be advantageous due to the community-based nature of the role.
- Applicants must be able to satisfy the relevant safeguarding and background-check requirements.
