Responsibilities
- Develop, review, and update comprehensive care plans tailored to each client’s individual needs and preferences
- Coordinate with care teams, families, healthcare professionals, and service providers to ensure seamless delivery of care services
- Supervise and support care staff, providing guidance on best practices and ensuring adherence to care standards
- Monitor client progress regularly, adjusting care plans as necessary to optimize well-being and safety
- Oversee transportation logistics, including driving clients to appointments or activities when required
- Maintain accurate documentation of all care activities, incidents, and client interactions using IT systems
- Lead on safeguarding procedures, ensuring all staff follow protocols to protect vulnerable clients
Skills
- Proven experience supervising or managing care teams within a senior or care home setting
- Strong leadership qualities with the ability to motivate and guide staff effectively
- Excellent communication skills for liaising with clients, families, healthcare professionals, and team members
- Competence in developing and managing detailed care plans that meet regulatory standards
- Valid driving license with the ability to drive clients safely when needed
- Proficiency in using IT systems for documentation, scheduling, and reporting purposes
- Knowledge of safeguarding policies and procedures relevant to vulnerable adults
