This is an exciting opportunity to shape integrated neighbourhood working across West Berkshire, helping health, social care, voluntary sector and community partners deliver more joined-up, preventative and responsive services. The role will support the development of sustainable neighbourhood models that improve outcomes for local people and align with local system priorities.
About you
We are seeking a confident and collaborative programme lead with experience of transformation, integration or service improvement across health and care systems. You will be skilled in stakeholder engagement, programme management, communication and facilitation, and able to use data and insight to support decision-making, manage risks and drive continuous improvement.
What you will do
You will lead and co-ordinate integrated neighbourhood working across West Berkshire, supporting sustainable operating models, multidisciplinary ways of working and the use of population health management to inform priorities. Working across organisational boundaries, you will bring partners together, monitor delivery, manage risks and support change that enables care to be delivered closer to home.
Fixed term role until 31/03/2028 (Funding limited project)
We are ideally looking to offer the successful candidate the full number of hours advertised above. We may however consider offering less hours than advertised providing they meet the requirements of the role and service which will be determined by the recruiting manager.
